CDC Attestation Form

Jesse, I filled one out ahead of time July 2021 leaving Greece - my first post-quarantine international trip. But having since returned from Croatia (Oct), Prague (Dec), and Barcelona (Mar), United, American, nor Lufthansa have wanted it. Last month, I just quit thinking about it…..

Flying TO Rome you need your CDC vaccination card and PLF
Flying home FROM Rome you need a negative test result

If you mean the PLF then yes they will have at your airline check in

Can you just print it out before your trip and fill it out later? Or are you in Rome already? Ask your hotel to print it out for you. When I traveled in Sept. Swiss Air had the forms at the airport as no one seemed to know they were required. I had printed mine out at home before I left the US.

NOTE: US citizens do not need to fill out Section 2.

ChristineH, the CDC attestation form is the form needed to enter the US. It is required in addition to proof of negative test or recovery. Whether all airlines actually check for it, who knows.

Thanks- I’m in Rome at an Airbnb so printing is not really an easy option. Was hoping they would have it there. I could electronically fill it out?

The last two trips no one ever wanted one. On the previous 3 trips they were handed to me at check in. Dont worry about printing.

I forgot about Mexico, so the last 5 trips including today, about 5 minutes ago, no Attestation Form.

We struggled at our hotel in Athens to print the form out. It turns out that the hotel internet was out, so the time at least was well-spent in identifying the problem. No-one at any airport we went through asked to see it, however.

KLM passed out a single page form in Amsterdam yesterday returning to Atlanta, and collected them before allowing boarding.

We had the CDC form (6 pages) in our backpacks, but this was simpler.

Any update on this? Seems stupid. returning on United 5/26. It looks to me like I would only need to complete Page 2 as a fully vaccinated US citizen?

Jeff, nothing has changed. Correct on just needing to fill out page 2 / Section 1. Not sure if anyone will ask for it. But, it’s a simple thing to print out and fill out just in case.
