Self Declaration Form for Travel to Italy from Abroad

I saw an article from CNN dated today, April 7... https://www.cnn.com/travel/article/italy-travel-covid-19/index.html
that Italy is requiring a self declaration form to be turned in to the travel carrier before entry. It doesn't seem like a big deal, but I have not seen this form mentioned in previous forum topics. It is not the PLU form.
Has anyone any experience with using/needing this form?

Never seen or heard of this and there have been plenty of forum members who have traveled recently to Italy, some are there now

We are leaving in 16 days and nothing about this in our Travel Ready section on United

If it’s not mentioned here I’d ignore it

Christine H, this seems to be an old form since all of the dates in it are in 2020. I haven’t seen it, and I am also preparing for a trip to Italy. Hopefully someone from Italy or a current traveler can comment.

We’ll see but I doubt it
There has been so much discussion here about entry requirements and it has never been mentioned AFAIK
I’m not fussed about it til I see it in an official government site
Wouldn’t be the first time CNN is wrong
( it’s a travel puff piece)

Apparently CDC has updated isolation guidelines, no longer need to isolate after testing positive so what would be the point of testing to return to US?
Haven’t had a chance to get to CDC site and confirm
I think test requirements will be dropped very soon

The form was required in 2020 and 2021 when I returned to Italy. I didn't have to have it this year.

After just returning from 3 mostly wonderful weeks in Italy, my memory is a bit vague on whether I had to show proof that I had filled it out in order to check in for the flight to Milan. However I did fill out the form online, it's quite easy and can be done days in advance. Basically they want to know where to reach you in the 24 hours after arrival in case a Covid case is discovered on the flight.

I believe you are referring to the PLF
that is different from the form that is mentioned in the CNN article
