Are you looking for one guide to take your through multiple cities? I assume it will be easier to find local guides/drivers just for areas closer to their home bases, for instance by getting references from each city's local tourist office. See also this similar discussion.

We went to Poland a few years ago with the intention of doing some searching for ancestors and used them. They were able to link us up with a tour guide in the Warsaw area (who was fantastic and we remain in touch with even today) and in Poznan. Very easy to work with. I highly recommend reaching out to them.
