We are going on a RS tour and listed on the itinerary is a cultural performance - concert or Black Light Theater - what do I need to pack for this? We each have a RS carry-on (small) so space is an issue. Any advice is appreciated...Thanks!

There are little concerts like this all over the place in Prague so they are very common and very casual.

I can not speak to this tour specifically, but there have been concerts/events where athletic shoes were not allowed. I always take a pair of sandals or nicer shoes just in case.
