Have any of you traveled by train with a dog in Italy? Is there a definite policy? I have all the usual paperwork.

I would start with the Italian Embassy website, and this website has some info: https://it.usembassy.gov/embassy-consulates/embassy/sections-offices/fas/pet-travel-faqs-italy-u-s/ Your paperwork looks to need to include EU paperwork as well.
As for general travel with an animal, Europe is more tolerant of dogs in bars, restaurants, trains, etc.; but I am not sure they have the same concept of emotional support/service dogs that has developed in the US. That means that museums, churches, and other official venues may not allow your dog in. That is just my observation, not based on experience.
Pets are indeed allowed.

We travelled to Italy for four weeks this past fall and used only trains and have no memory of ever seeing a dog (emotional support or otherwise) either on the trains or in the train stations. For that matter I do not recall seeing them on the streets, in restaurants or other venues. I am sure if you were in a residential neighborhood that would be different but thinking back on our trip it was dog free.
Can't rem if it was in Italy or not, but do remember seeing a dog on a train - he was curled up on the floor - a good sized dog! I'm sure I recall reading one time that you have to buy them a ticket - maybe it's discounted, maybe it's the price you would pay for a child, but not sure that it's free - could depend on the size of the dog and how much room they take up.
One of my happy memories was when we were in Venice in the early evenings and seeing the people out letting their dogs run and play in the neighbourhood squares. Not so happy memories were the strays hanging around Pompeii and Ostia Antica. (Just because I hate seeing stray dogs - it makes me sad)
I was on the Frecciabianca to Rome last year, and I was talking to this really interesting older woman, who was wearing tons of bracelets and turquoise jewelry, when this little dog came wandering by. "Can you just stay with me for a few minutes?" she asked the dog in Italian. The dog did for a bit, she rolled her eyes and said, "honestly, he just wants to be everyone's friend." So the answer is you can, don't worry about it.
