Of course all states have different ways of storing vaccine information for individuals.
Even though I have my paper copy I thought it a good idea to have a document that is directly from a state agency that has the records.
I live in North Carolina and now have a PDF document with a QR code which can be scanned.
i only got the white cdc card. curious to know how to get the document you are talking about. thanks.
My understanding is that all vaccine providers in NC send the data here.

When I got the vaccine they asked me to provide an email address.
The email address generated a username to login to the site. The user name is your email with covid19vaccine added at the end.
You should be able to see the records and also print the PDF with the QR code which validates the information.

I looked into getting something other than the CDC card, but so far, no luck. My state (Maine) doesn't seem to have any centralized registry, and since I got my shots at a mass vaccination clinic put on by a hospital at which I'm not a patient, I don't seem to be able to get it through the vaccine provider.
Re Maine. I would be surprised if the vaccine records are not stored in a state health site but it may take some research. I had to do quite a bit of research to figure out how to access the NC records. I had to contact my doctor, who had no idea and the local health department that actually administered the shots and finally the state level .

I got my shots in South Dakota. I get no health care there. But I wrote an email to the state's department of health asking for anything they had. Within 24 hours they sent me a screenshot of my vaccination records. (I think it was a screenshot since it was a png file.). I printed it out.
So, for those who want a copy of their vaccination record, and don't know who to contact, send an email to your state's health department. You never know.

I'm in Oregon and I also got my shots at a general vaccination clinic that was not connected with my normal health care provider. However, the record of my vaccination was somehow transmitted to my provider and is now part of my health record there. Have you checked with your regular health care provider to see if they have the record of it?

Also, I just got a statement from Medicare with the dates of my vaccines which is just another document that might help. Who knows.
It doesn't help the majority on this forum, but if you are in the UK, your "Vaccine Passport" appears if you have the NHS app on your smartphone - all the data plus an encrypted QR code.

I read an article that stated that each state has a record of immunizations. Apparently whenever a healthcare provider gives you a vaccine, it is reported to your state. I live in Maryland and was able to find mine.
This is the website that the CDC publishes. You might have to do a little digging on the links that are provided. I can confirm that I was able to find the webpage, register, and look at all of my immunizations in Maryland. For me, Maryland did not provide a QR code on the download of my record.
