My sister and I are traveling to Italy from USA in September. Are there any specific vaccinations required for travel documents ?
No, there are not.
No, nothing required medically from US to Italy (or anywhere in Europe that I'm aware of).
Basically, if you don't worry about it here, don't worry about it there. But, of course, it's always a good idea to be up to date on normal vaccines (tetanus from a rusty nail is a risk here, Italy, and everywhere else)

Note that they never recommend anything less than this - Italy has no more suggestions than Canada.

As mentioned, it's a good idea to ensure that routine vaccinations are up-to-date for travel in any part of the world. You may also find it helpful to have a look at THIS website for their information on Italy. In addition to routine vaccinations, Hep. A is also a good one to have for travel anywhere (IMO).
