Because we are taking a transatlantic cruise at the start of our vacation and have seen people (too frequently) airlifted off the ship at what appears to be an exorbitant fee, plus our age in country, we are looking at travel insurance. Has anyone used IMG (International Medical Group)? If so, what kind of feedback can you provide? Other suggestions?

Give the folks at The Trip Insurance Store a call. They do a great job of advising what vendors to us, what you do or don't need and general advice.
While we have purchased policies from IMG, we have never had a claim. I have used Insure My Trip to compare policies. You can contact them via phone, email or chat with any questions.

You will see that Allianz is popular for medical & trip insurance. Medjet is popular for medical evacuation since it will get you home instead of to the nearest facility. Look at both websites to see if these will work for you. (Although I have bought both, I have not filed a claim.)
IMG was recommended by a tour company I used a few years ago and I have used them since. I have only filed one claim which was in late 2024. They provided the expected reimbursement amount.
We use MedJet for evacuation and American Express for international health insurance. I don't purchase any additional trip cancellation or interruption insurance.
For health insurance, unless you travel abroad a lot, a term policy is usually gonna be better than a yearly plan.
Can I ask which American Express card you have?
