MXP is in the Lombardy region. You need to specify exactly which area or town of Lombardy you need to reach. Lombardy is a large region about the size of Massachusetts.

If it is near the Duomo, you can take the Malpensa Express from the airport to Milano Centrale or Milano Cadorna. Then you could take the metro to the Duomo stop, but at that point I would take a taxi.

Malpensa Express is the name of the train line: http://www.malpensaexpress.it/en/. It runs frequently and costs 12 euros. Buy tickets at the station and date-stamp them in the machine before boarding the train. Dubrovnik airport has a regular bus to the Old Town at Pile Gate or you can choose a taxi; see also www.airport-dubrovnik.hr. Guidebooks also cover the airport transfer issue.
For travel to that part of Milan, I'd suggest using the Malpensa Express to Cadorna station rather than Centrale. Fare is ~€10 and that trip will be about 35 minutes. You can buy your ticket at the MXP airport rail station. From there you can easily take a Taxi to Piazza del Tricolore.
Note the the M.E. has two routes, one to Cadorna and one to Centrale, so be sure to board the correct train. Also you MUST validate your ticket prior to boarding the train or you'll risk hefty fines, which will be collected on the spot!
Actually, the Piazza del Tricolore is most easily reached from Milano Centrale using the No 9 tram if you want to take public transportation. Exit the station on the right hand side looking out from the station. I believe there is a side entrance. You will be in a small park called Piazza IV Novembre. Across the park is a street called Via Fabio Filzi. The tram runs on that street, head south direction Porta de Genova. Takes takes you right to Piazza del Tricolore. You can buy a city day pass at most tabacchis (newsstand, ciggy shop).
Or take a taxi. The Piazza del Tricolore is equidistant from Cadorna station or Centrale station. The trains to Cadorna are faster because they make only one or two stops. If you plan on a taxi, I'd just take the first available train from Malpensa. The departures alternate on approximate 15 minute intervals.

If you take a taxi once you reach the station, then get whichever train gets you to Milan first. The Malpensa Express to Centrale takes longer than Cadorna, but if you have to wait longer for the Cadorna train, then you might as well take the Centrale.
