Your question is a wee bit too general and open ended - asking about entire countries needs some fine tuning based on your interests (which are... ?) Otherwise you're going to get really generic advice that's all over the board. I'm from Poland and it's a huge country - you can spend a week there, or two, or three, or an entire month and not see everything. I'm sure the same can be said of the others, although they're smaller. I would hit the library, do some research first, identify what interests you, and then mock up a rough itinerary based on your priorities. You'll get much better responses if you have more specific questions (after you've already done some research and at least made a list of priorities). Travel books have good sample itineraries in them already, so why reinvent the wheel?
We did a week long bus tour of Poland several years ago. Recommend Gdansk, Warsaw, Krakow and the Auschwitz death camp, as well as a few other cities.
In 2011 we did a tour that included Russia and all three Baltic countries. We spend two nights in each country. It was a bus tour.

You have all of Sept or most of it for this trip to Poland and the Baltic countries? What luck!!
I would find it easy to put together an itinerary because I most certainly know what I want to see in this whole region...numerous places to recommend in Poland, both urban centers and small towns/villages too. On my 2nd and 3rd trip I went to a village and small town...no problems, easy to do taking the bus and the train.
It comes down to what your priorities are in visiting Poland as to the places. I'll be doing a day trip this time to Poland in May from Vienna too see a village south of Katowice...doable given the train times.
My suggestion is to focus on Poland and Lithuania, culturally and historically, although Estonia is well worthy of your time.
