My wife and I are traveling to Paris and Italy for 21 days in March and are looking for some help with itinerary. We fly into Paris and plan on 5-6 nights then heading to Milan for 1-2 nights, after that we are looking for some ideas. We have been to all the big cities in Italy and are looking to do something different and off the beaten path. The only thing we have to do is be in Naples about 2 weeks from leaving Paris. We thought maybe a night or two in Lake Como and then a few nights doing in Tuscany visiting wineries, perhaps with a guide. We are not renting a car , so trying to do this by train. Any suggestions would be appreciated!!
Could you fit in Turin (Torino) and/or Genua (Genova) between Paris and Milan? Or, instead of staying in Milan, in nearby Pavia? Or put in Bologna before setting off to Naples?
In Naples we stayed in the Vomero neighborhood. We liked it very much since it is a quieter pleasant neighborhood with easy metro access. There are also excellent restaurants and bakeries and many parks.
Vomero is beautiful. A couple of nights in Bologna could give you a day in Ravenna.
Second the suggestion for Torino.
How about Lago Maggiore instead of Como?
Or, Paestum or Salerno?

Paris 6 days, Milan 2, Florence 6 days with side trips to siena, montepulciano,Pisa, Sorrento 4 days and then Naples for the last few. It looks like trains are available for all this.
Or take a train to Salerno for a few days as a base to visit the Amalfi Coast and Paestum.

Good plan overall. You will find Montepulciano from Firenze a stretch as it is about 3 hours each direction by train. I seldom recommend driving, but cutting Firenze to 3 nights you could rent a car to stay 3 nights near Montepulciano and enjoy some Tuscan countryside. Glad you dropped Lago di Como. March is not yet the season there.
