We've used this site. It allows you to plug in what you want and then compare 6 or so different plans from various insurance companies. Sometimes for the same or a little more, you can increase coverage dramatically.

We like Divers Alert Network (DAN). http://www.diversalertnetwork.org/membership/travelassist.asp For $55/year, you get family membership that covers mainly evacuation (up to $100,000) but also some medical assistance, anywhere more than 50 miles from home. We've never needed to use it but have a friend who did, and it worked just as advertised.
