No, but you might as well be up to date on your tetnus shot--in case you cut yourself somehow you would have peace of mind if you couldn't get to your regular dr. right away.
No there are not. However, it is a good idea to carry a business card from your doctor with their fax number and email address just incase you might need to contact them from Abroad. That is standard travel advise applicable to travel in general and not specific to London and Paris.
I strongly agree with Matti's suggestion. It is advisable every ten years. I think you can get a Booster for Tethnus, Diptheria and Polio?? Protection against Hep A & B is also good to have.

No, unless you've been to a country that has yellow fever. Then you might need to show proof of vaccination. But that affects on a very, very few people.
I'd also be sure to check that you have sufficient measles & mumps coverage. Both are making a comeback, particularly in the UK. Side effects can be nasty - especially for men - and it would certainly mess up your vacation if you got infected.
