Help for disabled travers at airports and train stations

Europe is not like the US where so many place are handicap accessible. While most airports are modernized train travel is quite different. I would try to plan as far in advance as possible. Many train stations have few if any lifts (elevators) if you need to change trains it can be very difficult.

I think it would be helpful if you let us know what countries and what cities. Some are better than others. I know Lufthansa has a great service to assist travelers with disabilities, and you can make arrangements with the Deutsche Bahn for assistance too.
