The cheapest and most convenient continues to be a debit card and an ATM.
If you're a member of AAA, check with them for their rates. Also, check your credit union.
I concur with Frank, ATM and a debit card. If your bank charges high fees for foreign transactions, find a different one. My small local credit union charges nothing additional.
BofA had the best rates for smaller amounts of cash (1000-2000$) and no fee for 1000$ or more. Can pick up at your local branch. Cheaper than the mail order or mall.
I've just purchased mine for June from www.foreignmoney.com of Beverly Hills, CA. They were 2% less than Bank of America on the fee (Exchange rate). Also, they will deliver to your office (not just your home address).

The comments about best rates and no fees means nothing unless you also post the exchange rate. The exchange rate is everything. A recent poster contacted me privately to criticize my posting saying that banks will bury fees in the exchange rate. Her bank was not charging her any fees and promised her she was getting the same rate as an ATM would give her in Europe. The rate ???? 1.43 when the interbank rate that day was 1.34. A good deal for everyone.
