My husband and I (both in our 40s) are planning a month long trip to Europe. Due to my walking difficulties, we need to take a wheelchair. I have a lightweight transport wheelchair, which we have taken throughout the US, Mexico and Canada. However, because of the cobblestone streets, I think I need a different wheelchair, or at minimum, different wheels. I would appreciate your thoughts. Thank you.

Can't recommend one but if you google "Off road Wheelchairs" you'll see some like you describe.
This guide is one result. It's split into manual and motorized versions. The manual versions start off by listing replacement Wheels to get you over the rough areas. That might be enough for you.
