My husband is in the process of renewing his, and is receiving some contradictory instructions. The passport application says to staple the photos to the application and in another place it says to do it differently (ie, NOT stapling the photo) but does not give any specifics. It's been a couple of years since I renewed mine and I cannot remember what I did. Has anyone here renewed lately and did you staple or not staple the photo?
If he's using form DS-82....you are right in both areas. You staple one photo to the application but don't attach the other.

I renewed my passport a year ago, and went to Europe last August.
The name on my passport has my middle name, my airline ticket did not. No problems.
I submitted two pictures, as directed, one loose, one stapled to the form. I don't know if this was a mistake, but they returned both pictures, including the one with the staple holes in it. I think they scan the picture, put a copy of the image on the passport and another in the file.
