So if you have 4 people, living in three households how/can you get insurance so that if something happens to one of the 4 the whole trip can get covered if it gets cancelled.
Not sure if that is possible or not.
Any ideas?

Probably not possible. Lets say 1 couple has to cancel for a covered reason, medical say, the other 2 continue and do the trip. If any common costs are incurred, like a vrbo rental, the cancelling couple gets reimbursed by their insurance for their share.

It depends. If you are all on one policy, you should be covered if one person cannot make the trip for a covered reason.
Av few years ago we booked a ski trip to Whistler with another couple. 4 people, one trip. We had to prepay for the condo and lift tickets as a package. I had never bought trip insurance before, but for some reason I did this time. On the morning we were to drive up to Whistler, the other man ended up in the ER with a pulmonary embolism and we canceled the trip. As we were all 4 on the same trip insurance, we were fully reimbursed for the expense of the trip we had to cancel.
