My husband and I are leaving for Spain on May 8th for the RS May 12-22 My Way Tour. In anticipation of our trip, I downloaded the SpTH app on my phone to start the health entry form, knowing I wouldn't be able to complete it until 72 hours before our trip. Although I put the dates in correctly, they kept shifting to incorrect dates and wouldn't allow me to correct them. As a result, I decided to fill out the form online manually and didn't have any issues. I received an FCS code via email. Now I am a bit paranoid though that everything will revert to the wrong dates somehow. Has anyone else had these issues?

I have heard of a few people having problems when filling out the form via the app, similar to what you described. I have started the process online and haven't encountered any issues so far. I have to wait until next week though to complete the process as I don't leave for Spain until May 6. I initially started the application for the QR code the last week of March. I checked my account today, and the information I had entered before was correct.
Of course, I have also heard of some people having some issues who started the process too early, so hopefully I won't encounter any problems next week!

Just an update for the OP, I leave for Spain on Friday and completed my application for the QR code today online. Everything went fine and I received my QR code within seconds of submitting the form. Hope everything goes well for you and have a great trip!

Oh boy, can I relate. We are arriving in Spain on 15 May. The app keeps changing my arrival date to 14 May and won't let me edit it. So I went online to start a form instead and it literally took 12 tries to log in after I registered. The site kept sending me a new confirmation code email, then it changed my password to the confirmation code, or the IOS suggested password, grayed everything out and I had to start over.
I am also concerned I won't be able to complete this form in time. Did anyone else have problems registering online and signing in again?

I can't say I encountered any issues. When I first registered on the site I received the 6-digit numerical code, but I never once had to use it. I always just entered using my own password.
Every time you access the site, you are sent a 6-digit code, but that seemed to be more for verification purposes. Hope you have success in completing your form!

I've had the same issue while using the iOS app - it changed both my birthdate and my date of arrival to one day earlier. I was debating if I should just enter in one day later for both of those sections, but was worried which date would actually appear on the final form. Switched to the online website and no problems so far.
