We are traveling on Thursday to Milan (SAT-EWR-MXP). When filling out this form I listed 'direct" flight and just showed EWR to MXP. Did I do this right? Or do I need to list the SAT-EWR portion? Also, it only showed JFK as a departure airport not EWR so I chose other and then listed EWR. Please help me figure out if I did this correctly. Thank you!
I am changing planes in the US and included the domestic leg thinking that if one tries to track with the airline they typically track from the 1st leg of the ticket.
A related question to add: Should I be entering my flights to Italy and the flights back to the US? Or only the info for how I get there? I filled out the online form including all the flights & it emailed me a confirmation, but it looks like it says I arrive in Rome on the day that I will depart. Expect this may be user error on my part!
The directions at least imply including all legs on your itinerary there. So yes, probably should have included the Seattle-EWR leg.
You are correct, the form dates from the days of Covid flights, so only ATL and JFK (DELTA) are included in the drop down, click "other" and then a box appears to enter an airport manually.
No return flights need to be included, the last leg entry should be your arrival flight at your first Italian stay, so even if you stay in Rome for a day, then fly elsewhere in Italy, that domestic flight should not be included, unless it was an immediate flight from Rome.
However, not sure what they do with the forms, I believe they are just backup in case an airline reports a positive cae on a flight. Delta only mentioned the form before boarding, I indicated I had it, they did not review it, no one looked at it on arrival, but I changed planes in AMS, so was on an EU flight coming to Rome.
Thanks Paul! I am trying to get it right before I go to avoid scrambling to update forms when I get there. Passport control after a long flight can be tiring enough without adding more to do. My assumption is that this is basically for contact tracing purposes in case of need.
THANK you Paul. I tried to go in to edit and it will not let me put in the dates and times correctly. Not sure what to do. I am going to try one more time, but then might just have to go with it as is and hope for the best.
Jacque_smithiii. I filled out our forms using exactly the same reasoning you first used. We are on that same flight in a few days from Newark. I think there is no need to worry about editing to include your prior US flights.. As I understand it, the form is now entered digitally in their database and provides a way to do contact tracing if necessary. Italy is concerned with where you are coming from in the US and where you are entering Italy. Also, of course, you must also have a Covid test to show along with your CDC card.

Thank you - I think I am "over it" all. I have uploaded all my information to United's website and we are checked in for our flight tomorrow. Praying everything is on time and we make our flight in Newark. We have a tight layover. Hope your trip goes well.
