We are headed to Italy in a month. I know we'll have to show our CDC card to enter anywhere but does my passport also have to be shown at the same time? Should I have a lanyard that will hold both? Not sure of the protocol for entering museums, restaurants, etc. If I need a lanyard to hold both, does anyone have a link of one they have actually bought and used?
I've read that Italy's current Covid rules are subject to expire at the end of March, though the current, omicron-era rules have been extended a number of times. So just a heads up that it could all be different next month.
I don't have experience with Italy, but when I was in parts of Spain that called for vaccine proof at venues, the picture of the CDC vax card on my phone sufficed. I never carried the card nor my passport. The "Italy" part of the Destination Q&A forums might have better guidance there.
I wouldn't want to be turned away at a venue I wanted to enter, so I'd carry both the card and my passport. If I wanted to live really dangerously, I might try to get by with a color copy of the photo page of my passport.
Having traveled to Italy in November of 2021, I was asked for a CDC card and Passport at restaurants and museums.
I held on to both documents for my husband and myself in my money belt. (Yes, I still use a money belt :) ). im not comfortable with a lanyard, but that’s just me. I just wanted to make it easy and quick for myself.

I'd not feel comfortable with important documents like that on a lanyard unless you've got them tucked in your shirt. I just carried my CDC card and passport in my purse in a zipped and secured inside pocket. (Secured closed with a carabiner).
I also generally use a money belt as well but during the October trip (to France not Italy) did not want to take the chance I'd need to get in/out of the money belt in public so stored them in my purse.
I always carry my passport - never on a lanyard - terribly uncomfortable and obviously insecure. If I think I'm going to need to show it it will be in an accessible but secure pocket. If I don't need it will be under my clothes. If you need a photo ID to go with your CDC card I'd take a driver's licence and keep that with the card - lighter, and easier to replace
