Attestations?

I apologize in advance if this question has been asked and answered. There are so many threads regarding re-entry into the US and the Covid rules, I may have missed it. We will be returning to the US on United Airlines. There it mentions that all passengers must provide a document called an "attestation" along with their negative Covid test. Apparently, this is a requirement of the government and not United. Has anyone had experience with this?

I don't know about United. We just flew back on American, and American used Verifly. All the attestation amounted to was a checking a couple of boxes and agreeing that you do not have covid per testing or other. Using the app, it is part of the package and the airline sees the completed package and you are permitted to check in. Without online ability, I imagine it is signing and dating the forms at the check-in counter.

Easy peasy.

If you have not already, I'd strongly suggest downloading the United app. When time comes to do online check-in, you'll upload your vaccination card and COVID test results. When I've uploaded mine, the vaccination card was a JPG and negative test results were as a PDF. The attestation (form) was made electronically as part of online check-in.

I recently returned to the US from Paris on Air France.
I was given the paper attestation form when I checked in to drop my suitcase and it was collected at the gate to the plane.
There were extras for people who didn't have them.
It's a requirement but not a big deal.

I haven’t been asked for one since July, 2021, and that is a return fo the U.S. from 4 different countries.

I just returned from Greece about two weeks ago. I was surprised when I saw that I needed the attestation. I had my hotel print it out for me. It’s seven pages long but I basically had to fill out one page and sign another. One person looked at it. Other than that it was never asked for, but I was glad I had it just in case. I flew on United.
