Form for Entry to Turkey

This appears to be required and not the HES.
We connect via LHR. Do I put I've visited the UK on the form where it asks for countries in the last 10 days.
Do you get any proof they received or do you just print it?

I arrived in Turkey last night. I printed the HES form and my Turkish Visa. They never asked for anything except my passport. I am guessing that it’s all hooked in electronically via your passport, although I would still keep printed copies just as a precaution.

I don’t know how they treat connecting flights on the HES forms.

We connect through Germany to Istanbul. Since I have not "been there" in the last 10 days and will stay airside for transit through the airport, I did not put Germany in the form.

After completing the document, I downloaded it electronically. I printed a copy and uploaded the electronic version into United's Travel Ready center.

Not a soul looked at our form, though I imagine they could view it electronically. I can't imagine this happening since the only people wearing masks here appear to be tourists, probably American. (Unscientific observation on my part.)

@CWSocial, Turkey relaxed their mask rules a few weeks ago. I think they are only requiring masks on public transportation and medical facilities these days. I certainly haven't seen any masks to speak of this week.

I’m flying into Istanbul Thursday and connecting on a flight to Izmir before going on to Ayvalik which is my final destination. Looking at the “Form for Entry to Turkey” now and it restricts choices to a limited list of cities. Ayvalik is not on their list- Istanbul and Izmir are, in addition to perhaps 30 Turkish cities.
There’s a required line for “address.” It would appear the address should be the lodging address in Ayvalik.
However, the form does not allow any selection beyond its limited list of cities.

In some cases I found that the "city" was too small to be on the list and there was a larger nearby city that was part of the official address. I found these by googling the hotel property which would then list a complete address, including city and district.

Also, they're not always in alphabetical order.

Yes- the layout of the form itself is a bit illogical for those used to reading from left to right. A column on the left lists cities a person entering Turkey is supposed to select from as their destination. The list is incomplete. Even the major cruise port of Bodrum is not listed.. So, the second column lists regions in Turkey-which probably only Turks are familiar with. Without any instructions, a tourist is supposed to know that you need to select the region your destination city is in if it not among the cities in the ( incomplete) left column. Select a region in the center column( (in this case “ Balikesir” ) and then a new list of cities within that region will pop up for you to choose from, I found trying to remember how to spell Ayvalik enough of a task, that I never had the time to learn that it’s in a region I also can’t spell!
A very unfortunate form indeed, particularly if the government wants to encourage tourism to Turkey this summer.

As a late update, when we arrived, no one asked for passenger locator form or vac card - only visa. We had combo of American Airlines and British Airways flying here. Both checked vax card only (actually both asked for PCR test and I had to remind that card sufficed).

I uploaded the passenger form into United's Travel Ready center. No one asked to see the physical version and I'm not convinced anyone did anything with it electronically.

But then, cases are very low in Turkey and nearly all COVID protocols have been dropped.

Upon entering Turkey at IST airport, the immigration official only asked for proof of a visa- no health entry form, no proof of Covid vaccination or health insurance— just visa proof and I had purchased an E-Visa online which presumably he had access to.
Be sure to print your e-visa out so you can show the paper just in case of a digital problem.

My experience was the same as described by Kenko: the immigration official asked to see my passport and the paper printout of my eVisa for Turkey. I didn't try to show the digital version, which may have worked.
