There are 2 port options Vahy or Pythagorion from Kusadasi in the morning. The schedule appears to have returns from only Vahy? I assume if you want see Pythagorion, it leaves an hour later than Vahy, you have a taxi option or bus to get to Vahy for your 5pm return. Is there enough in Vahy to spend a relaxing walk around day? I am sure the hotel has great, up to date,advice but I enjoy reading stories from RS travelers.

When is your trip? I'm seeing a 5pm ferry leaving Pythagorio in October on ferryscanner.

Why would you need a visa? Greece is in the EU, you'll need your passport and will go through EES at the Greek ferry port if they have it up and running.
I don't have any answers to your question other than to note that some on my RS tour took the ferry on the day the tour ended and did not get to the terminal far enough ahead time. They were not able to get through security in time and missed the boat. They were able to take a later departure but it messed up their timing.

I found another listing from Meander Travels that includes returns from both cities. Maybe no visa but you do need to pass through immigration. Tilos ferry, not included in RS book, also maybe and option to get to Vathy.

Correct, you will go through passport checks going and coming back as you are changing countries and entering/leaving the EU/Schengen zone. Turkey no longer requires a visa for US citizens.
