The most recent rail pass I bought from ETBD Travel was enormously wasteful. It was sent UPS Second Day Air when my trip was more than a month away. A 41 cent stamp would have sufficed--even with delivery confirmation it would have cost under a dollar to mail.
Then there was all the wasted paper--a timetable with only Western European cities when I have a pass for Hungary, maps with only half of Hungary (and that half was so small as to be useless), and a DVD I have already seen. Even if I had not already seen it, how many times can I watch it? I sent all the material back, as I have done with previous passes.
Maybe ETBD Travel could cut the $15 fee to $2 if you skipped all the junk and the express mail.
I always order mine from RailEurope - they always have some sort of free shipping promotion going on - you still get maps and a timetable, but that's it!
