Does anyone know how to find the sight disabled Mod. 28 T/C form so that you can purchase the discounted tickets?

That discount is for the visually-impaired (blind). For the mobility-impaired, there is the Carta Blu, but I doubt a non-EU citizen can get one without a lot of hassle.
