What is the best way to get from the airport to a hotel in Rome? How much does each way cost?

It would depend on how you define "Best." The best way is a Limo and a driver with little wine on the side. Probably do it for 50 to 75E. If by best you mean cheapest, then it is the Sitbus shuttle at 8E. In between would be a taxi at a fixed rate of 40E or the Leonard Express at 14E. With train or bus will need to walk or taxi to your hotel.

The Leonardo Express train will take you into central Rome (Termini). From there you have several choices depending on how much luggage you have, how far away the hotel is, and your physical/mental condition. Walk if the hotel is close to Termini, otherwise take a taxi.

Frank, Claudia, and Swan give you some good options. Currently the Terravision Bus Service is running a special for €4 so it's the least expensive option. The Cotral Airport Bus comes in as the next least expensive at €4.50 - then the SITBus Shuttle at €8. Not sure where your hotel is located but if you're in Trastevere or Ostiense you could ride the The FR1 Metropolitan train for only €8. Next on the cost chart would be the most commonly used train to the city, The Leonardo Express . Today, it costs €14 per person to ride this train to Roma Termini. A taxi to the the Fixed Taxi Rate Zone in the center of town is currently €40 and then, as Frank mentions, there are many shuttle options - at various prices.
