Can I get a train from Melk (or Durstein or Krems), Austria to Prague? Approximately what is the price one-way? How long is the trip and how many trains are there per day? Okay to make reservations one or two days ahead?

According to Deutsche Bahn website the trip takes almost 7 hours and you have to change trains at least twice. There are many trains. You can choose the train which suits you most on DB website.

Here is a case where I would recommend using the Austrian Rail website, www.oebb.at. It will show routes including bus connections. For some of the routes, such as the 6h55m one leaving Melk by bus at 9:45, you can get online ticket for the rail section (Gmünd to Praha hl.n.) for only €39 in 2nd class. The buses, Melk to Krems and Krems to Gmünd would be extra.
