You take the train that goes to those places. You may have a choice of speed between the two places -- a high speed train with fewer stops but at a higher price. An Intercity with more stops and somewhat slower. And final a local regional train that hits all the stops. Go to bahn.de for general schedule information or for the Spanish train site detail price info. The Spanish site is a little hard to use because it will not stay in the English mode. But Spanish might be easy for you.

You're going to want the AVE (fast bullet train) for those routes. Madrid to Toledo is from Atocha station...less than 10€ and 30 minutes. The AVE to Barcelona is also from Atocha and takes about 3 hours. Price is above 100€. You might be able to get a discount on that route when purchased from the RENFE website in advance. If you don't mind the airport hassle, you can probably get a cheaper flight from Madrid to Barcelona on one of the budget carriers.

There are multiple (about hourly) trains to/from Toledo to Madrid Atocha Station. Some do fill up as its a popular commuting line and ticket lines at Atocha can be 90 minutes. Be sure to buy a round trip ticket for that to guarantee a return seat. Once in Toledo, there is a local bus that takes you to the center or an escalator up the hill. Madrid to Barcelona is about a 3 hour trip on the high speed line leaving from Atocha. Buy in advance online (Renfe) to get the best price. Flying Madrid to Barcelona doesn't make financial or time sense in your case.
