Rental car insurance in Italy

After reading Rick's advice and many posts on car insurance in Italy I am still wondering if this is the best plan. We will arrive on rail at the Pisa airport to pick up a rental from Avis. (would it be cheaper in a nearby town and if so, which one?). We will not decline the CDW. Should we purchase the Super CDW there or buy a policy here in the US (maybe Travel Guard)? Is there anything that can be done about the large amount of money if the car is stolen that is not covered by "Theft Protection"? We rented in Italy 2 1/2 years ago and I don't remember this being so complicated. Thank you for you input on this much-discussed but confusing subject.

I just don't want to get caught in any fine-print or procedural problems with third-party insurers. Also, with third party insurers, if something happens you pay first and seek reimbursement later. In other countries, I use Amex's premium car coverage. Only $24.95 per rental for first-dollar coverage. But unfortunately rentals in Italy are excluded (which gives you a clue about the frequency of claims in Italy).

Best advice I found prior to my first driving experience in Italy is here http://www.gemut.com. In the right hand column, above the picture of a winding road, click on "What you should know about renting a car in Europe." I found Gemut to have highly personalized service and they beat the AutoEurope online quote, as well as Hertz' quote. You can call and ask advice once you get your online quote, which will help reduce your confusion. Buon viaggio!
