From Pisa, you will start by going to either Verona or Milan. Pisa to Verona involves several changes, at Firenze and either Bologna or Padova. Pisa to Milan usually involves one change at Firenze, but there is a direct train through Genoa that takes a bit longer. You can go from Verona to Peschiara del Gard by train in 14 minutes, and take a boat up to Malcescine from there. Or from Milan, go to Sirmione (one hour) and take the boat up the lake from there.
