www.malpensashuttle.it shows bus departures from Milano Centrale FS at 3:45, 4:00, 5:00 and every 20 minutes thereafter.

The Malpensa shuttle bus takes one hour from Milano Centrale to MXP. Doesn't sound like it leaves early enough for you. As noted by the previous poster, if you really must be at the airport at 4am, you should stay at an airport hotel. Is the airport even open at 4am?
