In May I will be leaving from the Hotel de Ville in Milan for a Malpensa flight which leaves at 7:10 am. What would be the best mode of transportation (not including a 100 euro taxi ride!) to use? I am assuming I would need to leave Milan around 4:30 am. Thanks for any information.

First, ask your hotel for a recommendation. Second, most cities have a shared van shuttle company. After that it is either the bus or train but I don't know how early they started.

Jean, I'd probably use the Malpensa Shuttle that departs from Milano Centrale. A departure at 04:15 would get you to the airport at 05:05, so that should be early enough for your flight. The fare is €7.50, which is considerably better than a €100 Taxi ride! Your Hotel appears to be about 2 kM from Milano Centrale, so you would need to take a Taxi to the Station. Another option would be the Malpensa Express which has a departure at 04:28, with arrival at MXP at 05:04. Fare is currently €11.00. Your Hotel appears to be closer to Cadorna station, so this might be the best option. I'd suggest asking the Hotel to pre-arrange a Taxi, so that you'll won't have any problems getting to Cadorna. The first Malpensa Express train out of Cordona departs at 04.23. The second at 04.53 The trip takes 36 minutes.
