Our flight leaves Malpensa (heading back to the US via Paris) at 11:15 AM on a Wednesday. I have not been in this airport in about 20 years. Is the standard three hours before the flight enough? (Always nervous about missing a plane!)

You won't have to go through passport control in Milan (you'll do that in Paris), so three hours seems excessive to me. A flight from Milan to Paris is essentially a domestic flight. I'd probably do two hours myself.
Agree with Andrew. It is a domestic flight - two hours should be more than adequate. You might go a little earlier and have breakfast after checking in if concerned about time.
