we would be arriving at Milan (Lampugnano) bus station around 8pm and wondering what the best way to get to MX P would be? do we need to go to the central station first? thanks

I've never had to make this connection but I would go to google maps to figure out distance. I see it takes 32 minutes by metro just to get to Milano Centrale. Personally, I'd probably budget for a taxi.

No, in your case it is faster to go to Cadorna (using Metro line 1, which serves Lampugnano), and take a train to Malpensa from there.

Yes, exactly what Andre said. And the malpensa express from cadorna to the airport is about 35 minutes.
