Depends on the train. Some reservations are required...some are optional. Seat reservation does require an additional fee. There are some trains that you cannot make seat reservations.

Just to clarify the above, if a reservation is required (e.g., for Eurostar in Italy, TGV in France, AVE in Spain), the reservation is included. If a reservation is not required but is available, it will be extra. In Germany, if purchased with a ticket it's EUR 1.50, otherwise EUR 3. Some trains do not accept reservations (e.g., RE/RB in Germany, R in Italy).

Yes, if the train requires reservations, you will still need to get one at extra cost. Arranging them can be difficult since the number alloted to Rail pass holders is limited.
