Well, the train from Frankfurt to Zurich only takes about 4 hours, so that would be my choice, rather than the whole go to the airport 2 hours early to wait on your flight, the flight itself, then once you land, take the train from the airport into the city. Besides, you get to see some scenery on the train. If you are going to be in a county, you might as well see it. Check the Deutsche Bahn website for trains to Milan, to see how long they take.

I'd take a train between Frankfurt and Zurich, and fly between Zurich and Milano.

I see that there are direct trains from Zurich to Milan every couple hours, and it's actually a shorter trip than Frankfurt to Zurich (3hrs 41min from Zuerich HB to Milano Centrale). For one date I picked in October, the only direct flights Zurich-Milan were Swiss Air, and the prices were sky high. So train definitely looks like the way to go on this route as well.
