Yes, technically it is the International Drivers Permit and it is accompanied with your state's drivers license. Have to have both.

YES it is mandatory.
get it at aaa.com/IDP, same day ($30, 2 passport photos, good for 1 year) have it before you go

To drive legally, yes.

The rental agency may or may not ask to see it.

The police certainly will.
In the one week I was driving in Italy I went past 2 check points where police were randomly waving cars over. In one case it was the car directly in front of me. No body had violated any traffic laws, just a 'we want to check' stop. My recollection is that the fine for not having it is in the 'hundreds'.

Get the IDP. I was stopped at a random back road in Tuscany. The police officer was young spoke no English. He checked it and off we went.

Remember you are in a foreign country. While many speak English, it would presumptive to assume everyone does. It’s would be like a European expecting North America police officer to speak or read their language.

As the others have pointed out, an IDP is compulsory for driving in Italy. Just to clarify it's not a license but rather a permit, which has to be used in conjunction with your home D.L. The rental company may or may not ask to see it, but if you can't produce it for police you may face hefty fines, which may be collected on the spot.

If you haven't driven in Italy before, you'll also need to be aware of the limited traffic / ZTL areas within many cities and towns. Many of these use automated cameras, while others rely on local police. Passing through these zones will result in hefty fines which you probably won't know about until you receive notification after you get home. There are also speed cameras including the "Tutor" which measures both instantaneous and average speeds. Fines can be significant, ranging from small amounts for minor infractions to over €500 for high speeds, and they are sent directly to the car owner (including rental car agencies, which then charge admin fee.

According to the UK Government website UK citizens do not need an IDP to drive in Italy. Of course this may be a European exemption and it may be compulsory for other nationalities. This thread has just led me to check as I have a car booked in Sicily in 2 weeks and do not have an IDP. My immediate panic is over!

When getting an IDP you should note that there are three variations and any specific country may require any of the 3. There are 1926, 1949 and 1968 variations. Should you need one Italy requires the 1968 version.
