Considering using the Dolomite Airport Shuttle from Milan airport to a hotel in Ortisei. It is very affordable and seems too good to be true. Has anyone used this shuttle service?

I can't give feedback as I have not done this, but I also can't figure out which company you are referring to. Is there a specific shuttle?

Ask your hotel to see if it knows this company. Double check with the Ortisei tourist office.

I have not used this company, but at almost 200 Euros per person, it seems steep to me and it is not exactly door-to-door unless you are staying at one of the few large hotels they stop at.
We go Milano to Ortisei on the train to Bolzano (change at Verona) and have Taxi Ivan pick us up at Bolzano. He charges about 100 Euro one way. http://www.taxi-ivan.com/eng/index.htm. He will take you to the door of your lodging.

Laurel—-have you used Taxi Auto Ivan recently? The request section of the web form only goes from 2015 to 2021, and cannot be moved to 2023. A Google search does not find anything more recent.

Whenever we travel in the mountains, we will always rent a vehicle in order to have the freedom to see the best vistas, etc. We always see so much more and find our travel so much more efficient..

We last used Taxi Ivan in Sept 2022. I just email him through his site. Recently some friends booked him for their time in the Val G this autumn.
