Before flying back to the US from Venice recently, I filled out some sort of attestation when checking in with the American Airlines app. Since a Covid test was no longer required, it was basically just some question about my Covid vaccination status or something. I have Global Entry so didn't need to fill out anything else or find any special form.
I haven't seen one of those in about 4 trips now.
But not to worry, if you need it they will give it to you at the counter and it takes 15 seconds to check a box and sign
Nope, not needed at this time. In the course of checking in online for my Delta flight I was asked my phone number for contact in case there was a documented exposure on the plane and they needed to notify you.

A couple of days before returning from France a little over a week ago, I got an email from American Airlines with a link to a Contact Tracing Form they require people to fill out.
