I think the most appropriate Eurail pass would be a 5 day Select 3 country (Benelux, Germany, Poland) Saver Pass for about $400. The Czech Republic is not included in Eurail passes.
You have only shown four days of travel so the pass works out to $133 (€86) per day. Only one leg, Munich-Amsterdam would be completely included in the pass.
If you are willing to ride in 2nd class you can probably do a lot better P2P. The German part of Praque to Munich can be done using a €27 ($21 per person) Bayern-Ticket. For Munich to Amsterdam, I found Dauer-Spezial fares as low as $59 for next week (they should be available at €39, $60, if purchased farther in advance).
You can also get a reduced rate on EuroStar with a valid Eurail pass, but the pass does not cover the trip.
I would thoroughly investigate what you can get in point to point first before deciding on a pass. Use the national rail companies (DB, Dutch Rail, Polish Rail, etc) websites, not RailEurope, for fares.
That is what I thought, P2P was cheaper when I went from London, Brussels, Paris.
I also book about 50-60 days in advance anyway, to be safe. I wish the tickets went to Czech and England.
