Is 16 minutes enough time to transfer from a Regional train from Monterosso to a Eurostar to Rome? Also, the trenitalia website says reservations must be made in Italy for the Eurostar train. Does that mean I have to wait until I get to Italy to purchase and reserve?

Hi Kim. 16 minutes is more than enough. Actually five minutes is plenty, it's a small station. The potential problem is that while the intercity trains are quite punctual, the regional trains from Mont to La Spezia are often late; you will want to have a plan B. Not sure where you saw about reservations must be made in Itally. You can easily buy the La Spezia to Rome ticket online. Enjoy your trip!

May be what you mis read or mis understood is that you must have a reservation for the train. So if you were to miss the connect in Monterosso, then you would need a new reservation.
