Our Visa covers collision/damage to rental cars when booked and paid for. In Portugal I understand we have to carry proof of insurance. How do we get this if paying for it stateside?

Heather, your car insurance probably does not cover you in Europe, therefore proof of insurance is likely a moot point. We have ours with a large company and we are not covered in Europe. I always ensure that my credit card covers us, and that's it.
