Hi,
I would like to go back to England and take the tour that includes Scotland/Wales/Ireland, and London, but would not do part of the tour in London. I have already been to those places included in the tour but would like to visit other ares of London. Is that possible?

Yes, it's fine to skip activities in the city, as long as you tell your guide in advance. However, we don't have one single tour that covers all those regions. You can take more than one tour, back-to-back, if that's your plan. https://www.ricksteves.com/tours/england-scotland
When we did the Heart if Italy last year one if the couples skipped out on the Vatican etc because they had been there before no issue just let your guide and buddy know so they don't wait for you.
You're a paying customer, not a prisoner. Relax. Just let the guide know so the entire group is not left wasting time waiting for you to show up.
