Does anyone know which flight needs to be used on a two leg journey getting to Lisbon? We leave sfo on a United flight via Munich. In Munich we board a Lufthansa flight to Lisbon. Not sure which flight to use on the form. Plus I don’t see Lufthansa listed in the drop down box for the airlines, which I find very strange.

Use the flight info for the flight that actually brings you to Portugal - the last leg.
I flew to Lisbon from Frankfurt last August, on Lufthansa. Look again on the form, it should be there (we had no trouble). The airline code for Lufthansa is abbreviated "LH".
David thanks for the help. I filled out the form today with no issue. I did finally find Lufthansa listed under Deuetche LufthanSA.Ag in the drop down. I am glad you made me look deeper.
