We're planning on going on the southern Italy tour. The return flight from Naples leaves at 7:40am. Will it be difficult to get to the airport in Naples and check in on time? We will have to go through customs and are worried about what time we will have to leave the hotel. We don't want to have to leave at 4am or some crazy hour.

If your flight is going somewhere in Europe, treat it like a domestic US flight (2 hours). If it is direct, you still don’t need a full three hours because the airport is small.
At that early hour, taxi can take as little as ten minutes.
I hate getting up early far more than most people, but I could handle a 7:40 out of Naples.

Naples airport opens at 0330. Check in for direct intercontinental flights open 3 hours before departure. If you will be connecting with another flight at another Schengen airport, then check in would be 2 hours before departure. In neither case would you have to go through Customs. You will have to go through Passport Control in Naples if you are on a direct flight, as well as a security check.
If your hotel is anywhere in the center of Naples, a taxi would only take 10-15 minutes or so, that early in the morning. Technically you need to be at the airport by 0440 for a direct flight to the US. 0540 for a flight connecting at another Schengen airport.

We spend every Christmas in Napoli. Rather than get up dreadfully early to take a 6AM connecting flight home, we sleep until a reasonable hour, take the 70 minute high speed train to Rome and fly home direct (to ATL) from FCO on an early afternoon flight..
