This is where we have stayed that fit your criteria:
Berlin - Pension Peters
Salzburg - TOP Hotel Mozart
Munich - Hotel Mark and Arthotel Munchen (we liked the Arthotel a little better)
Munich: I have stayed at Hotel Uhland on a couple of different trips and have always been pleased with it.They offer good week end rates. The breakfast is wonderful and the area quiet. It is, however, a bit of a walk from Marienplatz and some people don't like that.
Salzburg: I have stayed at Pension Bergland, but it has been a few years. We really liked it, but after 2002 it no longer fit into our budget. We now stay at Haus Reichl on Moostrasse. It is about 2km outside the old town but a bus stops a block away that goes right into Salzburg. We love the owners and the quiet location and at 52 euros a night for a double it was perfect. The rooms are large and spotless. Have a great trip.
Munich: Stayed at the Hotel Ludwig, opposite the train station. Tourist class hotel. Friendly staff, nice rooms, great breakfast.
We like staying at Pension Peters in Berlin--basic but liked the friendly staff and the location near public transportation.
I'm not sure what you consider "nice." We're budget-minded but require ensuite and breakfast. These are the places we stayed in Munich and Salzburg. Weisse Taube is within walking distance of the castle (well the BASE of the castle!) and shopping.

In Salzburg we have stayed at the Golden Ente 3 times and loved it each time! It's in an awesome location right in the old town 1/2 block from the action. It's a little pricey, BUT - they have a two family room that is priced like it's a budget hotel. They're double goes for 125, but the "apartment" goes for 180 a night. I highly recommend it!

In Munich I usually stay at Hotel Uhland and would definitely recommend it. The breakfasts are incredible and it's located in a quiet residential area. While it is a bit of a walk to Marienplatz, it's only about 10 minutes or so to the Munich Hbf, so not hard to get anywhere in the city.
In Salzburg I stayed at Hotel Goldene Krone and would also recommend it. It's in the "new" part of the city but I found it to be easy walking distance to most of the sights, in a nice area and the owners were wonderful.

'nother vote for Pension Peters! The people were SO friendly and the price was excellent. I am PICKY about lodging and no complaints (except a little bit noisy from downstairs bar from our particular room location.)
I guess I am alone here with thinking that an apt. for 4 would be nice and reasonable at the same time. It will certainly cost less than a hotel. We used this website to find a lovely little apt. for me and my daughter and the cost was only 48 € per night. Separate entrance, fully equipped kitchen, a couple of stops from the main train station and very near the metro and the Tiergarten. We really liked this option.
The website is in English, lets you pick out your neighborhood, shows you availability, says if the landlord speaks English, all the apt. amenities and photos of course. We found the site to be professional and thorough.
