Like Tom, I love the Hollander Hof in Heidelberg. I have been to Speyer as well, but have not stayed there overnight. If you would like a possible option for Worms, i have stayed there @ the Dom Hotel, on Obermarkt 10. It's was not as charming as the Hollander Hof, and is not a B&B, but it may be a lesser priced option for you. They have a small restaurant there as well, and i recall the food was very good.

Speyer, Worms and Heidelberg are all within less than an hour's drive of each other. Are you planning to stay in all three, or are you just shopping around? In descending order of preference, I would rank them Heidelberg > Speyer >> Worms. The only one I've stayed the night in is Heidelberg, in the Hotel Hollander Hof, right by the old bridge. It was very nice, but not too cheap.
