We are planning a 10 trip April 15 to Belgium/Netherlands and are finding most of the B&B's are asking for cash. We have always used credit cards at our accommodations in our European travels to avoid carrying a lot of cash. What is the current best way of exchanging currency? Amex offices, ATM's ....???

If you find a place that accepts a credit card, you are paying too much.
Most small, family run B&Bs don't accept credit cards. It's only the large, chain hotels, that already charge too much, that do.
The simplest way is to either bring cash from US, buying Euro before you go. My credit union charges the day rate plus .50 cents for the stamp to mail the letter. OR get enough to cover what you need from the ATM all over the airport or from any bank ATM, located all over the city. Be sure to have enough to cover takeouts in checking. You cannot move money or take money from savings from ATM in Europe. Also, be sure to tell bank you are traveling or they will lock up your ATM account.
