It's pretty strait-forward. Lots of people do it. Everybody seems to enjoy it. We did.

If you happen to already have a rail pass and want to use it for the rail portion of the Nutshell, you will have to make seat reservations ahead of time, and pay extra for it. But the other three legs of the trip can be done on the fly, without reservations and without a tour. The bus, boat, and cog train don't fill up. Just pay upon arrival.
